Serial carotid MRI identifies rupture of a vulnerable plaque resulting in amaurosis fugax.

چکیده
A 66-year-old man with cryptogenic ischemic stroke and multiple acute ischemic lesions in the right middle cerebral artery territory was enrolled in the CAPIAS trial (Carotid Plaque Imaging in Acute Stroke; NCT01284933). Baseline carotid MRI demonstrated a right-sided nonstenotic American Heart Association type-VI plaque with a large lipid/necrotic core and intraplaque hemorrhage (figure 1). Eleven months later, the patient presented again after an episode of right eye amaurosis fugax. Repeat carotid MRI revealed a new ulceration on the right side with large parts of the former lipid/necrotic core missing (figure 2). We hypothesize that plaque rupture had caused embolization into the right retinal artery. Figure 1 Baseline high-resolution carotid MRI after cryptogenic stroke

MRI Plaque Imaging Detects Carotid Plaques with a High Risk for Future Cerebrovascular Events in Asymptomatic Patients
PURPOSE The aim of this study was to investigate prospectively whether MRI plaque imaging can identify patients with asymptomatic carotid artery stenosis who have an increased risk for future cerebral events. MRI plaque imaging allows categorization of carotid stenosis into different lesion types (I-VIII). Within these lesion types, lesion types IV-V and VI are regarded as rupture-prone plaques...
متن کاملUse of Antiplatelet Agents Is Associated With Intraplaque Hemorrhage on Carotid Magnetic Resonance Imaging
Atherosclerosis in the carotid arteries is an important cause of ischemic stroke. Treatment decisions for carotid artery stenosis are currently based on the degree of luminal narrowing and the presence or absence of recent ipsilateral stroke or transient ischemic attack (TIA). However, there is increasing evidence that additional factors may be useful in stratifying a patient’s risk of stroke r...
متن کاملDetection of Unstable Carotid Plaque by Tissue Doppler Imaging and Contrast-Enhanced Ultrasound in a Patient with Recurrent Amaurosis Fugax
Ultrasound (US) is one of the most important diagnostic tools available for the detection and evaluation of carotid stenosis. The case of a 70-year-old woman with recurrent right-sided amaurosis fugax presented here highlights the way in which tissue Doppler imaging (TDI) and contrast-enhanced US (CEUS) may aid in the diagnosis of carotid plaque vulnerability. Furthermore, the novel inverse fly...
متن کاملAtherosclerotic plaque ulceration in the symptomatic internal carotid artery is associated with nonlacunar ischemic stroke.
BACKGROUND AND PURPOSE Atherosclerotic carotid plaque ulceration is considered a marker of previous plaque rupture and subsequent thromboembolism. It can be accurately detected with multidetector CTA. We hypothesized that atherosclerotic plaque ulceration is associated with nonlacunar ischemic stroke rather than lacunar stroke. METHODS Prospectively, 750 consecutive patients with transient is...
متن کاملAmaurosis fugax associated with congenital vascular defect
A 68-year-old female with no significant past medical history presented with loss of vision in the lower half of her left eye that lasted <5 minutes. No abnormalities were found on ocular or physical exam. Computed tomography angiography and carotid ultrasound were performed, which confirmed the diagnosis as amaurosis fugax with two abnormalities leading to the transient retinal vessel occlusio...
